Induction-maintenance trials in which HIV-infected patients switch from standard three- or four-drug therapy to two NRTIs, a PI alone, or an NRTI plus a PI have shown high virologic failure rates. In recent studies of simplification to ritonavir-boosted PIs, most patients have maintained virologic suppression, but the trials have been small, and the durability of suppression is uncertain. With once-daily dosing and few of the lipid side effects of other boosted PIs, atazanavir/ritonavir alone is an attractive possibility. How potent is this regimen?
