Viral loads are routinely used to monitor the effectiveness of antiretroviral therapy (ART) in North America, even though the clinical implications of virologic failure are uncertain. Using data from 17 North American cohorts, the NA-ACCORD group evaluated the incidence of virologic failure on a second ART regimen, along with associated mortality.
A total of 36,188 patients initiated ART between 1996 and 2005; 17,820 of them experienced a first episode of virologic failure (defined as ≥1 viral-load measurement >1000 copies/mL, measured >3 months after starting or switching to an ART regimen containing ≥3 antiretroviral agents).
