Boosted protease inhibitors (PIs) are a highly effective component of many modern antiretroviral regimens, but they can interact with numerous drugs and are associated with various adverse effects, most notably metabolic events. In the SPIRAL study, investigators explored (yet again) whether patients with virologic suppression on boosted PIs can safely switch to raltegravir.
The multicenter, open-label trial (partially sponsored by the maker of raltegravir) involved 273 patients on boosted PI–based regimens who had viral loads <50 copies/mL for at least 6 months; 139 were randomized to switch to raltegravir, and 134 continued their PIs.
