Several studies have documented poor clinical outcomes among HIV-infected patients whose CD4-cell counts do not improve with antiretroviral therapy (ART) despite sustained virologic control (a phenomenon known as immunologic discordance). Few studies, however, have evaluated the clinical significance of such discordance among patients who only recently started ART.
In this population-based cohort study, researchers reviewed data from more than 2000 patients in Canada who had sustained viral loads <50 copies/mL within the first year after starting ART. At 1 year, 19.9% of the patients were found to have CD4 counts <200 cells/mm3, and at 2 years, 10.2% continued to lack immune recovery despite continued virologic suppression. This latter group…
