Assembling a suppressive antiretroviral regimen for patients with multiclass drug resistance is challenging. Treatment guidelines suggest the use of two (or preferably three) fully active antiretrovirals, but this goal is often unachievable. In such situations, clinicians often consider adding nucleoside reverse transcriptase inhibitors (NRTIs) to the salvage regimen, even if these agents are only partially active or high-level resistance has been demonstrated. The advisability of such an approach, however, is uncertain.
