In June 2026, the U.S. Food and Drug Administration approved the use of tebipenem pivoxil (Utebzi), an oral carbapenem, for the treatment of complicated urinary tract infection (cUTI), including pyelonephritis, in adults who have limited or no alternative oral therapeutic options.
Clinicians have increasingly relied on carbapenems to treat cUTI because of a steady rise in extended-spectrum beta-lactamase (ESBL)–producing Enterobacterales in health care and community settings. ESBL-producing organisms are resistant to all oral cephalosporins and are often coresistant to quinolones and trimethoprim–sulfamethoxazole, leaving patients with cUTI with few or no oral treatment options.
