During pregnancy, maternal and neonatal outcomes are optimal when maternal circulating glucose is between 63 and 140 mg/dL. Patients with type 1 diabetes who rely on standard diabetes management are outside this range about 50% of the time. New closed-loop insulin delivery (CLID) systems link a continuous glucose monitor to a smart insulin pump. In an open-label trial conducted in Canada and Australia, researchers randomized 91 pregnant people with type 1 diabetes (at <14 weeks’ gestation) to a CLID system or to standard care. Standard care was a continuous glucose monitor plus either an insulin pump or multiple daily insulin injections.
