Observational data have linked artificial sweeteners to type 2 diabetes and worsened glycemic control, and additional studies have suggested a plausible pathway, involving alterations to the gut microbiome and glucose metabolism. In this study, researchers conducted a randomized trial of 180 people with type 2 diabetes (average age, 60; average glycosylated hemoglobin [HbA1c], 7%) who regularly consumed artificially sweetened beverages. Participants were assigned to drink 24 ounces daily of either an artificially sweetened beverage or unflavored bottled water. Both groups were provided with their assigned beverage (adherence was >90%), and those in the water arm were advised to avoid artificially sweetened beverages.
