Patients with anti–N-methyl-d-aspartate receptor (anti-NMDAR) encephalitis typically present with rapid behavioral changes or psychosis. About 80% substantially improve or fully recover with first-line immunotherapy (IV glucocorticoids, IV immunoglobulin, and/or plasma exchange) and second-line agents (rituximab or cyclophosphamide), but some have prolonged disorders of consciousness characterized by unresponsive wakefulness.
