The largest and longest trial comparing screening colonoscopy and fecal immunochemical testing (FIT) is COLONPREV — initiated in Spain in 2009. Nearly 60,000 people 50 to 69 years of age were randomized to an invitation for either a colonoscopy or biennial FIT and followed for 10 years. Importantly, among those invited to have colonoscopy, only 20% actually underwent the procedure, whereas about 40% of those invited to FIT did one or more rounds of FIT ().
The intent-to-screen analysis examined outcomes by original invitation, and not by whether the person actually underwent colonoscopy or FIT. In this analysis, 10-year colorectal cancer (CRC) mortality was the same in both groups (0.2%).
