Plain radiographs are often the first imaging study performed in children with suspected intussusception to exclude bowel obstruction or perforation. In a retrospective review of plain abdominal radiographs from 73 known cases of intussusception (mean age, 17 months) and 73 age- and sex-matched controls, two pediatric radiologists who were blinded to diagnoses were asked to identify the three most specific radiographic findings for intussusception (intraluminal mass or intussusceptum, nonvisualized air-filled cecum, and obscured liver margin or right upper quadrant mass) and to estimate the probability of intussusception based on interpretation of radiographs alone. A probability of 50% was used as the threshold for diagnosis.
