A 3-day course of trimethoprim-sulfamethoxazole (TMP-SMX) is the recommended treatment for uncomplicated cystitis in women. However, fluoroquinolones are now replacing TMP-SMX as treatment for this condition (Journal Watch Infectious Diseases Apr 7 2006). Nitrofurantoin is an alternative to both TMP-SMX and fluoroquinolones, but its acceptance is limited by the recommendation for 7 days of therapy. In a study partially funded by the makers of the drug, investigators performed a randomized, controlled trial to compare shorter-course (5 days) nitrofurantoin with 3 days of TMP-SMX for uncomplicated cystitis in healthy women aged 18 to 45.
