The introduction of uniform criteria for systemic inflammatory response syndrome and sepsis in 1991 has facilitated research on sepsis, but since then, several limitations to this classification system have been recognized. A more sophisticated system is needed, taking into account parameters of predisposition, severity of infection, physiologic response, and organ failure (PIRO). In a recent industry-supported study, a multinational team of researchers developed such a system using data from 840 placebo-treated participants in a trial of activated protein C for severe sepsis (PROWESS). They then validated the system using a large global registry of patients with severe sepsis (PROGRESS; 10,610 patients).
