Cryptococcus neoformans is a significant opportunistic pathogen in immunocompromised individuals worldwide — particularly those with HIV infection, in whom meningitis is the most common manifestation. The closely related Cryptococcus gattii has also been recognized as a pathogen; it primarily affects HIV-uninfected persons in tropical and subtropical zones.
In December 2004, after an outbreak in Vancouver Island, Canada, a case of C. gattii infection was reported in Oregon; another case followed in 2005. In 2006 and 2007, 12 additional cases occurred, prompting the CDC, its British Columbian counterpart, and state and local public health authorities to establish the Cryptococcus gattii Public Health Working Group.
