Cryptococcosis is a known complication of solid-organ transplantation (SOT). Most cases occur from 16 to 21 months after transplantation, but a few develop within 30 days. The clinical characteristics of these very early onset cases have not been described. Now, a consortium of physicians who follow SOT patients with cryptococcosis have detailed their experience with very early onset disease.
A total of 175 cases of cryptococcosis after SOT were included in the multicenter cohort; 9 (5%) occurred ≤30 days after transplantation. Five of these nine patients (56%) had liver transplants compared with 43 of 166 patients (26%) who developed cryptococcosis later (P=0.05).
