Endothelial cell injury during sepsis leads to activation of the coagulation process through tissue factor (TF), resulting in capillary thrombosis and eventual organ failure and death. Tissue factor pathway inhibitor (TFPI) inhibits the procoagulant activity of TF. A recombinant TFPI, tifacogin, did not improve survival in a phase III study among individuals with sepsis, but subgroup analysis (Crit Care 2009; 13:R36) suggested benefit in patients with severe community-acquired pneumonia (sCAP).
