In resource-rich medical settings, fluid boluses of ≤60 mL/kg are recommended for children presenting with fever and shock. Nonresponsiveness to such treatment signals the need for intensive care. In a two-stratum study conducted at six centers in Kenya, Tanzania, and Uganda, investigators assessed fluid-resuscitation strategies in 3170 children aged 60 days to 12 years who had fever, impaired consciousness, respiratory distress, and impaired perfusion.
