Most cases of coccidioidomycosis, a fungal infection frequently encountered in the desert southwestern U.S., either require no therapy or respond to fluconazole or itraconazole. However, alternatives are sometimes needed because of refractory disease or medication intolerance. Posaconazole and voriconazole, two newer triazole antifungals, are possible alternatives, but data regarding their use in coccidioidomycosis come only from case reports or small case series.
