Recent findings suggest that PCI has no advantage over medical therapy in reducing mortality and morbidity risk in low- or moderate-risk patients with stable CAD (Journal Watch Cardiology Mar 26 2007). Outcome data are scarce, however, on the management of patients with asymptomatic ischemia after experiencing acute MI. In this Swiss, multicenter study, 201 patients with a recent MI (<3 months) and silent ischemia revealed by stress testing were randomly assigned to receive percutaneous transluminal coronary revascularization with balloon angioplasty (PTCA; average of 2 lesions treated per patient) or intensive drug therapy alone. Baseline characteristics were similar in the two groups (mean age, 55; LV function preserved).
