Ethnicity and Incidence of Congestive Heart Failure

In a large U.S. cohort study, higher incidence of CHF in blacks than in other ethnic groups was largely explained by higher rates of diabetes and hypertension.

Congestive heart failure is a major cause of morbidity and mortality in the U.S. To evaluate the incidence of heart failure in different ethnic groups, U.S. investigators for the Multi-Ethnic Study of Atherosclerosis examined 6814 individuals of four distinct ethnicities — white (38.5%), black (27.8%), Latino (21.9%), and Chinese (11.8%) — and followed them for a median of 4 years. None of the participants had clinical cardiovascular disease at baseline.

CHF occurred in 79 participants (incidence rate, 3.1 per 1000 person-years). MI preceded CHF in 26 participants (33%), and 63% of participants with CHF had preserved LV function (LV ejection fraction of ≥40%). The incidence rate of CHF was highest in blacks (4.6 in 1000 person-years), follow…
