One of the most common consultations requested of cardiologists is to “clear” a patient for a scheduled surgical procedure. Such requests often lead to extensive testing and interventions that can delay surgery and do not necessarily improve patient outcomes. In the DECREASE V trial, as in previous studies, prophylactic preoperative coronary revascularization in high-risk patients undergoing major noncardiac vascular surgery was not associated with improved 30-day postoperative outcome (JW Cardiol May 23 2007 and JW Cardiol Feb 4 2005). The DECREASE investigators now report their long-term results.
