In the absence of sufficient outcomes data, current American College of Cardiology/American Heart Association/Society for Cardiovascular Angiography and Interventions guidelines designate primary percutaneous coronary intervention as a class IIb indication — and elective PCI as not indicated (class III) — in facilities without cardiac surgery backup on site. Nonetheless, performance of both primary and elective PCI is increasing in such facilities. In this study from the National Cardiovascular Data Registry (NCDR), investigators compared outcomes in 8736 patients who underwent PCI at 60 institutions without on-site cardiac surgery with outcomes in 299,425 patients who underwent PCI at 405 institutions with on-site cardiac surgery.
