In patients with organic (degenerative) mitral regurgitation (MR), mitral valve repair has been shown to produce better outcomes than mitral valve replacement. However, the relative benefit of repair is less clear in patients with ischemic MR. In a records review study, researchers examined surgical results in 370 patients with chronic ischemic MR, half of whom underwent repair, and half of whom underwent replacement.
Before surgery, valve-replacement patients had lower LV ejection fractions and higher prevalences of severe MR, heart failure, and renal failure than valve-repair patients.
