Off-pump coronary artery bypass grafting (CABG) was developed to reduce the risk for pump-related adverse events, including diffuse inflammatory response syndrome and stroke due to cannulation of an atherosclerotic aorta. In the Veterans Administration–based ROOBY trial, 30-day outcomes were similar with on-pump and off-pump CABG, and the longer-term need for subsequent revascularization was more common with the off-pump strategy (JW Cardiol Nov 4 2009). However, ROOBY had no expertise requirements for surgeons, raising questions about how well off-pump CABG might fare at more-experienced centers.
