Pruritus on nondiseased (noninflamed) skin (e.g., pruritus of renal failure)
Pruritus presenting with severe, chronic, secondary scratch lesions (e.g., prurigo nodularis)
They then categorized itch based on the underlying disease:
Dermatologic
Systemic
Neurologic
Psychiatric/psychosomatic
Mixed disease
Other or undetermined origin
