Severe aphthosis is uncommon, but it can be debilitating, affecting the ability to eat and producing unwanted weight loss. Recurrent aphthous stomatitis (RAS) may be a forme fruste of Behçet disease. Topical corticosteroids, topical amlexanox, topical tacrolimus, and intralesional corticosteroids have been used to manage the condition in some patients. Those with more severe disease often require systemic therapy, such as colchicine, dapsone, azathioprine, cyclosporin, or thalidomide, but many of these agents have potential for long-term toxicity.
