In this case series, researchers described their anecdotal experiences with the use of infliximab for management of 26 patients who underwent ileal-pouch anal anastomosis (IPAA) for presumed ulcerative colitis (UC) and whose diagnoses were changed to Crohn's disease (CD) because of postoperative complications and events.
Median time between IPAA and CD diagnosis was 4.5 years (range, 0.1-16 years). Diagnoses were changed from UC to CD because of complex perianal or pouch fistulizing disease in 14 patients (54%), pre-pouch ileitis in 5 patients (19%), and both fistula and ileitis in 7 (27%). Patients initially received 1 to 3 doses of infliximab over 8 weeks, followed by various maintenance regimens.
