Does Clopidogrel Increase Mucosal Damage when Added to Aspirin Therapy?

Apparently not. Endoscopic evaluation after 1 week showed similar damage with aspirin alone, clopidogrel alone, or the two combined in a small, randomized crossover study.

Both low-dose aspirin (LDA) and clopidogrel reduce platelet function and increase the risk for upper gastrointestinal bleeding. To evaluate whether the combination of these two drugs magnifies that risk, whether acid reduction from a proton-pump inhibitor (PPI) might mitigate any increased risk, and whether outcomes vary by Helicobacter pylori infection or CYP2C19 genotype status, investigators conducted a four-arm crossover study involving 20 healthy volunteers in Japan.
