Outcomes in sarcoidosis affecting the hypothalamic-pituitary axis have not been studied extensively. These authors identified nine patients with sarcoidosis, hormonal deficiencies, and MRI abnormalities of the hypothalamic-pituitary region. They describe patients’ endocrine dysfunction, MRI findings, and clinical course.
All nine patients had gonadotropin deficiencies, and four had panhypopituitarism; seven had diabetes insipidus. The authors could not correlate MRI findings with characteristic patterns of hormonal deficiencies. Although lesions found on MRI resolved with steroid treatment, endocrine function recovered only rarely during a mean follow-up of 7.5 years. Sinonasal sarcoidosis was present in five patients.
