White-matter hyperintensities (WMH) are striking findings on MRI imaging. WMH have well-established predictors (vascular risk factors) but surprisingly weak clinical effects, which are limited to statistically significant associations with dementia and gait disturbances. Some patients with severe WMH have no discernible deficits. Numerous studies have failed to discover a classic pathologic correlate of WMH; white-matter rarefaction has been the main, qualitative observation.
