Confirming a diagnosis of small-fiber neuropathy usually requires specialized electrodiagnostic testing or histopathologic evaluation of intraepidermal nerve fibers. Corneal confocal microscopy (CCM) of sensory fibers that are derived from the trigeminal nerve and innervate the Bowman layer has been used to identify small-fiber injury in diabetic neuropathy (Diabetes 2007; 56:2148). Now, researchers have tested CCM in six women with non–length-dependent small-fiber neuropathies. Three had neuropathy of unknown cause; one each had impaired glucose tolerance, Crohn disease, and Sjögren syndrome.
