Identification of ischemic stroke etiology is critical for appropriate evaluation in the acute setting and for selection of secondary stroke prevention. Current clinical methods of subtyping are imperfect: About 30% of strokes remain cryptogenic or of uncertain cause after extensive investigation. The potential of biomarkers of ischemic stroke subtype is frequently discussed, but current candidates lack the sensitivity and specificity necessary for use in clinical practice.
