Endovascular treatment for acute stroke remains controversial, because recanalization does not necessarily lead to good clinical outcomes. To examine the potential for use of imaging to guide patient selection and improve outcomes after endovascular therapy, researchers prospectively studied patients eligible to undergo endovascular treatment within 12 hours after an acute stroke. Participants underwent multimodal magnetic resonance imaging (MRI) before and within 12 hours after the procedure, and at hospital discharge (or day 5). The investigators used a computer program and blinded personnel to identify perfusion-diffusion mismatch (“target mismatch”) on the images.
