Bleuler’s conception that schizophrenia involves progressive cognitive impairment has received variable support during the past century. In this MRI study from Australia, researchers measured gray matter in superior temporal regions in 35 participants at ultrahigh risk (UHR) for developing psychosis, 23 patients with a first episode of psychosis (16 with schizophrenia), and 22 comparison subjects (mean age at first screening, 21). UHR was defined as mild or transient psychotic symptoms or as functional decline in the past year plus either schizotypal personality disorder or having a first-degree relative with psychotic or schizotypal personality disorder.
