Children with urinary tract infections (UTIs) often receive antibiotic prophylaxis to prevent recurrence. In a multisite randomized controlled trial, investigators in Italy compared recurrence rates associated with antibiotic prophylaxis or no prophylaxis in 338 children (age range, 7 months–7 years) who had first febrile UTIs, with or without nonsevere vesicoureteral reflux (VUR; grades I–III); 309 children had documented pyelonephritis as determined by dimercaptosuccinic acid (DMSA) scans.
