Most clinical trials of add-on therapy for uncontrolled asthma have been performed in adults, and the results have been extrapolated to children. Current guidelines recommend low-dose inhaled corticosteroids (ICS) as first-line therapy for all patients with persistent asthma. Recommended step-up therapy for young children (age range, birth–4 years) is medium-dose ICS and, for older children (age range, 5–11 years), either higher ICS dose or addition of a long-acting β-agonist (LABA) or leukotriene-receptor antagonist.
