A grossly positive resection margin (R2) after pancreaticoduodenectomy is a poor prognostic factor in patients with pancreatic adenocarcinoma. Some experts have proposed that a microscopically positive margin (R1) could be a biologic predictor of more-aggressive disease as well.
In this large single-institution study from M.D. Anderson Cancer Center, the natural history of 360 consecutive patients who were treated with contemporary multimodal therapy and who underwent pancreaticoduodenectomy with positive margins from 1990 to 2004 was evaluated.
