Colorectal cancer is the second-leading cause of cancer-related deaths in the U.S. For patients with stage III node-positive disease, adjuvant chemotherapy with FOLFOX (continuous-infusion 5-fluorouracil [5-FU] plus oxaliplatin [Eloxatin]) lowers risk for recurrence by nearly 50% compared with surgery alone. Treatment of stage II node-negative patients remains controversial, because, in clinical trials, adjuvant therapy with 5-FU has conferred less benefit for them than for stage III patients. In a recent update of the French MOSAIC trial, in which adjuvant FOLFOX was evaluated in stage II patients, survival benefits were limited to those with high-risk anatomic or pathologic features (2007 ASCO meeting abstract).
