Attenuated Immunochemotherapy for Older Patients with Lymphoma

Nearly half of patients older than 80 with diffuse large B-cell lymphoma were in remission 2 years after receiving reduced-dose chemotherapy plus rituximab.

R-CHOP (rituximab plus cyclophosphamide, doxorubicin, vincristine, and prednisone) administered on a 21-day cycle cures the majority of patients with diffuse large B-cell lymphoma (DLBCL). However, this regimen is poorly tolerated in elders.

To determine the efficacy and safety of rituximab plus a reduced dose of CHOP (R-miniCHOP) in older patients, investigators conducted a prospective, multicenter, phase II, single-arm, cooperative group study involving 149 patients with newly diagnosed DLBCL (age, >80; ECOG performance status, 0–2). Most patients had advanced-stage disease and intermediate- to high-risk international prognostic index (IPI) scores.
