Chronic glucocorticoid use is associated with osteoporosis and excess risk for fractures. In an international, randomized, double-blind, double-dummy trial sponsored by the manufacturer of zoledronic acid (Reclast), investigators assessed the efficacy and safety of a single 5-mg intravenous infusion of zoledronic acid compared with 5-mg oral risedronate daily for 1 year. Before randomization, all 833 participants (68% of whom were women and 40% of whom used glucocorticoids for rheumatoid arthritis) were categorized based on duration of glucocorticoid use (prevention group, glucocorticoids for ≤3 months; treatment group, glucocorticoids for >3 months).
