Although the first- and second-generation selective estrogen-receptor modulators (SERMs) tamoxifen and raloxifene are approved for prevention of breast cancer in high-risk women, neither agent has been widely used in the U.S. for this indication. Initial findings from the Postmenopausal Evaluation and Risk-Reduction with Lasofoxifene (PEARL) trial — a 5-year manufacturer-sponsored placebo-controlled trial involving 8556 postmenopausal women (mean age, 67) with osteoporosis — showed that the third-generation SERM lasofoxifene (0.5 mg daily) lowered risk for ER-positive breast cancer, coronary heart disease events, and stroke but raised risk for venous thromboembolic events (JW Womens Health Feb 24 2010).
