More than half a million unintended pregnancies could be prevented annually in the U.S. with correct, consistent use of oral contraceptives (OCs). The Contraceptive CHOICE Project is a prospective cohort study of contraceptive use in the absence of financial barriers. Researchers enrolled sexually active women (age range, 14–45) in the St. Louis area; participants chose OCs, patch, or vaginal ring and were issued cards that permitted free refills (available every 21 days for 3 years) at local pharmacies. A refill gap of ≥35 days defined nonadherent for that refill. In a study based on data from an 18-month period, contraceptive adherence was evaluated in 619 women (75% age 20–29; 57% black, 33% white).
