Although transvaginal ultrasound's role in detecting ovarian cysts and masses is well recognized, false positives associated with one-time identification of abnormalities have led to high numbers of surgeries per identified malignancy. To evaluate the performance of serial transvaginal ultrasonography, investigators have analyzed data from 1987 to 2012 involving >39,000 asymptomatic participants (median age, 56) in a university-based annual ovarian cancer screening program. After an abnormal screening sonogram, repeat imaging was scheduled at intervals from 6 weeks to 6 months. Women underwent surgery if the morphologic characteristics of the mass progressed, if its volume increased substantially, or if new-onset pain occurred.
