We know that the diagnosis of Crohn disease is often delayed by months or years from the onset of symptoms. Emerging concepts in Crohn disease research suggest that biologic agents, such as anti–tumor necrosis factor antibodies, and immunomodulatory agents can alter the natural history of the disease. However, treatment with these agents must be initiated before irreversible complications such as fibrotic strictures develop.
