Prevalence of human papillomavirus (HPV) — a major cause of cervical and anal cancers — is increased in HIV-infected women. Currently, the main approach to preventing these cancers is early detection and treatment of intraepithelial HPV lesions via Pap smears and colposcopy. Primary prevention via vaccination against the main oncogenic strains could greatly improve outcomes, but few data are available regarding the immunogenicity and safety of HPV vaccine in HIV-infected women. Previous studies of other vaccines such as those for hepatitis A and B have shown diminished response rates in HIV-infected individuals, particularly those with low CD4-cell counts or ongoing HIV viremia.
