High-definition colonoscopes using narrow-band imaging (NBI) have been tested for their capacity to improve adenoma detection in multiple randomized, controlled trials; results from both individual trials and meta-analyses were largely negative. A new high-definition colonoscope has a modified NBI system that provides brighter illumination (Exera III system, Olympus Corporation; also called the Olympus 190). Researchers in Hong Kong have now conducted the first randomized, controlled trial comparing the new NBI system with conventional white-light colonoscopy.
