Drugs probably account for most contemporary cases of acute interstitial nephritis (AIN). This topic is addressed in two new reports.
Mayo Clinic researchers describe 133 cases of biopsy-proven AIN in patients treated at their institution between 1993 and 2011. Fully 70% of cases were drug-induced; the drugs implicated most commonly were antibiotics (47 cases), proton-pump inhibitors (PPIs; 13 cases), and nonsteroidal anti-inflammatory drugs (NSAIDs; 10 cases). Twenty percent of cases were caused by autoimmune disorders; sarcoidosis was the most common disorder in this category (13 cases). Among 99 patients who had at least 6 months of postbiopsy follow-up, 47% had complete recovery of renal function and 38% had partial recovery.
