Henoch-Schönlein purpura (HSP) is commonly associated with disease in other organs, especially joints, kidneys, and gastrointestinal tract. Sometimes, only skin is involved. Factors associated with increased risk for systemic disease in children include older age, involvement of scrotal skin, severe abdominal pain, increased serum levels of immunoglobulin A (HSP is an IgA-mediated immune complex vasculitis), and relapse. Factors associated with increased risk for systemic disease in adults include purpura above the waist, recent history of infection, fever, absence of perivascular eosinophils, and summer onset.
