The estimated prevalence of type 2 diabetes in patients with cirrhosis is high (37%). Metformin is effective in the treatment of type 2 diabetes but has been reported to cause lactic acidosis and liver injury in patients with liver disease. Although the evidence for these complications is weak, some clinicians are hesitant to prescribe metformin in patients with liver disease, and some discontinue metformin in diabetic patients who develop cirrhosis.
To assess safety and survival with metformin use in patients with diabetes after cirrhosis diagnosis, investigators retrospectively assessed outcomes in 172 patients who continued metformin and 78 patients who discontinued the drug based on provider recommendation.
