Cognitive decline associated with stroke is a known source of social and economic hardship. However, the trajectory of cognitive decline pre- and poststroke in an aging population has been challenging to disentangle, given the variety of cognitive domains that may be affected by stroke and the associated medical comorbidities that also influence cognitive decline. In this prospective study, investigators followed cognitive status and incident stroke in 23,572 community-dwelling black and white participants 45 years or older for a median 6.1 years.
